  1. Cheques should be made payable to “University of Toronto”. 

  2. Please indicate the gift designation by citing “Duchenne Muscular Dystrophy Research Fund” either on a short cover note or on the memo line.

  3. Please include your full name, address and either a telephone number or e-mail address (or both). This information is necessary so that the University can process the cheque and issue a tax receipt for the donation.

  4. The cheque should be mailed to:
